Photocopy of Barangay Certificate of Residency<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">4. 1 pc 2&#215;2 recent and identical ID Picture with white background<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">5. 1 Long white folder<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">6. Certification of grade from the Registrar with an average grade of 2.5 (applicable for schools using numerical rating) or 80% (applicable for schools using the percentile rating)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">*If SHS Graduating Student &#8211; Photocopy of grades from Grade 11 &amp; Grade 12 (1st Semester only &#8211; to follow)<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">7. CCO Application Form.
