2. Photocopy of Certificate of Good Moral Character

3. Photocopy of Barangay Certificate of Residency

4. 1 pc 2×2 recent and identical ID Picture with white background

5. 1 Long white folder

6. Certification of grade from the Registrar with an average grade of 2.5 (applicable for schools using numerical rating) or 80% (applicable for schools using the percentile rating)

7. CCO Application Form. Please access the form through the provided link, print a copy, complete the required application, and submit it, along with the specified documents to the admissions office.
